WHAT IS HOME HEALTH?

Home health is a wide range of services provided to a homebound individual. Home health care is often less expensive, more convenient, and just as effective as care that is received in a hospital or skilled nursing facility.

Skilled Nursing

Our skilled nurses will assist you and your family in various ways:
 

  • Planning and coordinating home care needs and goals as advised by your doctor

  • Administering treatments and educating you or your caregivers on various care needs

  • Assisting in the monitoring of health improvement and stabilizing

Walking Aid

Rehabilitation

If ordered by your doctor: 

our physical, occupational, and speech therapists are available to provide in-home treatments to improve functional skills, advise on home safety, help improve independent function, and/or restore communication skills

Taking a Shower

Home Health Aides

Our home health aides assist you with short term intermittent personal care--these services often include: showers/baths, bed changing, dietary monitoring

Taking Notes

Social Services

Our social workers help to identify and obtain community resources and support for various needs relating to the patient's overall well-being.
Additionally, they are able to counsel patients and their families through the hardships and impacts of dealing with a diagnosis of illness or long-term care.
